You will schedule a phone consultation with one of our Intake Care Coordinator. Please complete the client intake form sent to your email PRIOR to the phone consultation. We will answer any questions you may have during the consultation We will check eligibility and obtain authorization with insurance if applicable or discuss private pay options.
